What is causing this? How can i prevent it from happening again?

There is a tear or cracks in the formed cabbage.

This could also be as a result of rapid temperature flux, cabbage can easily crack when exposed to rapid temperature change eg, if it has been dry for like a month with no serious rains and all of a sudden, heavy rains poured and a humid situation created that last for hours in a day time. This dis-stabilizes pressure in cabbage and cause cracking.
